Hospice Services Provided by Jansen

At Jansen Hospice, we offer our professional and personalized services in a home setting, a skilled nursing or assisted living facility, or in a local hospital. Each patient is different and requires a unique approach to care. Depending on their needs, they may receive one of the services listed below:

  • Routine Hospice Care – Hospice care provided in the patient’s home or place of residence
  • General Inpatient Hospice – Short-term hospice care provided in a hospital. This is when patient’s symptoms cannot be managed in any other setting.
  • Continuous Care – Time-limited benefit offered during a crisis. An LPN or RN stays with the patient for 24 hours to provide continuous monitoring, assessment, and medication administration. This must be approved by the Hospice Patient Care Coordinator and Medical Director.
  • Respite Care – When patients are transported from their home to a Medicare approved facility for up to 5 days to allow the primary caregiver respite (rest) from the daily stress of caring for a loved one.
  • Bereavement Services – Services provided to families for up to a period of 13 months following the death of their loved one involving counseling services and support groups with clinical social workers, specially trained counselors and volunteers who have experience dealing with all areas of bereavement.
